Cannabis sativa L has a long history of medicinal use. CBD and THC are the two components with the highest concentrations in Cannabis sp. The endocannabinoid system includes cannabinoid receptors (CB1 and CB2 protein coupled), their endogenous ligands (AEA – Anandamide and A-2G – 2-Arachidonoylglycerol), and the enzymes responsible for their degradation and synthesis. CB1 receptors are primarily expressed in the peripheral and central nervous systems, while CB2 receptors are highly expressed in the immune system. Up to 40% of the patients with cancer related pain have neuropathic components. CB1 and CB2 receptors are both upregulated in nervous structures caused by peripheral nerve damage, which could explain the positive effects of cannabinoid antagonists on chronic and neuropathic pain.

Chronic Pain/ Neuropathic Pain:

The most common indication for cannabis-based medicine (CBM) use is still to relieve chronic pain. The clinical evidence has found that there is a substantial evidence that CBM was effective for chronic pain treatment. Considering the current evidence, a panel of experts have recommended the use of CBM for neuropathic and nociplastic pain mechanisms too. They also suggested that CBM could be considered if there was a substantial inflammatory component and that CBD-only medicine could be tried initially due to its better safety profile.

Chronic Stress:

Stress causes considerable morbidity and economic loss that could be prevented by a safe, rapid and effective treatment. The stress response involves secretion of cortisol and catecholamines from the hypothalamus-pituitary-adrenal axis and sympathetic nervous system, and prolonged activity is a substantial causal contributor to many diseases, including nociplastic pain conditions and mental health disorders. Anxiety Disorders/ Depression:

Anxiety disorders generally involve an excessive anticipation of future threats that is accompanied by excessive fear and are the most prevalent psychiatric disorders. Preclinical studies support the efficacy of low dose of CBD for anxiety. A study on placebo controlled found that a single dose of CBD significantly decreased symptoms of performance discomfort, anxiety and cognitive impairment. An open-label clinical trial found that CBD significantly decreased depressive symptoms. Chronic Insomnia (Lack of Sleep or Inadequate Sleep):

Chronic Insomnia is one of the most prevalent complaints in primary care. Early work on whole plant cannabis showed that cannabis may help to improve sleep onset latency and sleep maintenance insomnia. An early-double blind randomized controlled trials found CBD in participants suffering from insomnia improved their duration of sleep significantly

The recommended dosing of CBM (cannabis-based medicine) is to start at a low dose twice a day (or once daily for insomnia) and gradually titrate the dose over two to three weeks until the participant’s optimal dose is discovered. So, it is desirable “to start low and go slow”. It is prudent to allow personalisation of optimal dose for effect and tolerability because the dose is dependent from person to person.

Dose may need to be adjusted as required for changes in indication severity, or changes in other medicines, foods or hepatic and renal function, heart related ailments that may affect CBM metabolism.

The side effects are dose dependent and vary from patient to patient.
